This Personal Questionnaire is designed to be completed separately and privately by you and your partner.

The purpose of these additional questions is to help me get to know you better, as you describe your partner to me. Gathering these additional details will help me create a Ceremony in a style that feels personal, authentic and uniquely yours.

Be sure to find somewhere relaxing to complete it and give yourself time to share everything you can think of.  Your answers will come direct to me when you select SEND at the bottom of the form.

You are welcome to answer all of these questions in as much detail as you are comfortable with.

Please let me know if anything you share is for my understanding only and you would prefer it not to be mentioned during the Ceremony.
